Setup:
Centers high, forwards low.

5 and 1 start with a side pick & roll.
4 sets a down screen for 3.
5 rolls down, 4 steps outside the 3-point area.
3 gets the ball from 1 for the 3 pointer.

Note that 5 already has rebounding position.

Variation:
1 can choose to play the other side.
