Play starts with a dribble entry on the strong side followed by a dribble hand off with 2.

At the same time 4 sets a screen for 3.

Pass from 2 to 3.

1 cuts hard under the basket to the other side.

2 gets a screen from 3 and goes to the middle of the bucket.

1 cuts to the other side using a screen from 5.

2 get a screen from 4 and gets free.

Most likely pass from 3 to 2 for a jump shot, don't forget to watch inside for 4,5 and the other side for 1.
