Player 1 passes to forward 3.
1 and 3 cross each other, using 5 as a screen and go to the baseline.
4 takes the guard position.
1 steps out and receives a pass from 4.
2 sets a screen for 3, if 3 is free under the basket 1 passes to him for the easy score.
4 and 5 set a double screen for 2. 2 goes to the top.
If 2 gets free 1 passes to him.
The play always works against lazy man defense.
Off course the (2) does not have to make the high low.
The midrange shot will actually be a better option, depending on the different players!
