2 uses the (staggered) screens from 5 and 4 to get free on the other side of the court.

1 passes to 2.

After the pass 1 cuts to the ball corner.

3 goes to the low post.

1 cuts to the other side, using a quick screen from 3.

At the same time the ball is swung from 2 to 4 to 5 to 3.

2 cuts to the corner were 1 just left.

End with a side pick and roll between 3 and 5.

Remember while playing this 1, 2 and 4 might be open for a shot.
